HCS Medication Reconciliation queries multiple nation-wide data sources, aggregates the results, and analyzes the medication list for interactions, duplicate drug therapies, compliance issues, and formulary interchanges. HCS Medication Reconciliation can be deployed as a medication history service, providing the aggregated home medication list of patients via paper or HL7 message, or it can be deployed with a the HCS Medication Reconciliation Portal, a user friendly and efficient tool for medication reconciliation documentation used through admission, transfer, and discharge. Deployed with the portal, a home/inpatient merged medication list presents a practical way for clinicians to perform and document medication reconciliation.
Acknowledging hospitals’ goals to receive stimulus funding through meaningful use, in September 2010 HCS tested for and received ONC-ATCB 2011/2012 certification for HCS eMR meaningful use software through Certification Commission for Health Information Technology (CCHIT®), an ONC-ATCB. HCS eMR meaningful use software is a workflow driven solution focused on high User adoption. HCS eMR builds on existing processes such as medication reconciliation, integrates with existing technology to streamline processes, leverages sophisticated rules and timely alerts to drive success and can be implemented quickly.
HCS eMR integrates easily with HCS eRx e-prescribing software to offer an easy-to-use, effective e-prescribing solution for physicians in the hospital setting. HCS eRx was awarded Surescripts® certification for Prescription Benefit messaging in 2010. HCS eRx can also be used with HCS Medication Reconciliation to provide physicians with e-prescribing capabilities at discharge as part of the medication reconciliation process, positively affecting the discharge process and increasing physician adoption of the solutions.
HCS’ other software solutions include both pharmacy and nursing focused applications. Our solutions, HCS MEDICS and HCS OrderImage, offer a streamlined approach to medication safety in the pharmacy. HCS MEDICS is designed to deliver access to patient information, allergy and interaction checking, and medication monographs, while HCS OrderImage further enhances pharmacy workflow by providing an electronic workflow optimization of written physician orders that enables both remote order verification and telepharmacy. Focusing on a nurses’ workflow, HCS eMAR provides a user friendly, safe method of medication administration documentation and using barcode technology enables verification of the 5 patient rights.
